Incident Narrative
On March 8, 2025, three employees were inserting U-tubes into a bundle. The task required them to alternate between standing on the floor and standing on a platform (about 36 inches off the ground) to push the tubes in; one employee would hold and push the back side of the tube, another would guide and push the right side of the tube, and the third employee would guide and push the left side of the tube. At about 7:50 p.m., they were having difficulty pushing the left side of the tube in and decided to remove the tube so that it could be reinserted. As the injured employee was pulling out the left side of the tube, it struck his face and caused him to fall backward off the platform to the concrete floor. He struck his head on the concrete floor and suffered a head injury.
