Incident Narrative
A temporary employee was extended 24 feet in a telescopic mast lift when the lift drove over a hole that had previously been filled with soil. The tires sank in the soil, causing the lift to tip over. The employee fell with the lift to the concrete floor. The employee was hospitalized with bruising to his head, face, and the left side of his body, as well as broken teeth, a fractured jaw, a fractured left arm, and a dislocated left finger. He required surgery. The employee was wearing PPE and fall protection was in place at the time.
