Incident Narrative

An employee was driving a side-by-side ATV in the yard of the facility to facilitate locking up for the day. As they were returning from locking up the gate, the side-by-side ATV hit a pothole in the yard causing it to tip over onto the passenger side. The employee sustained a broken right forearm.
