Employees were working to change a slide gate for a levee. They were unbolting a ladder from the concrete. The injured employee placed his foot onto the ladder and the ladder bounced off the anchor. The injured employee and the ladder fell 12-15 feet into the hole below. The employee sustained a dislocated shoulder, a fractured hand, and knee injuries.
