Incident Narrative

On March 22, 2024, an employee was working in the bucket of a bucket truck, about 20 feet above the ground, trying to remove a temporary power pole. The pole snapped and the employee was thrown, traveling 10 feet into the air before falling to the ground (for a total fall height of about 30 feet). The employee suffered broken ribs and a broken pelvis, as well as a punctured lung. Fall protection was being worn at the time of the incident, but the employee's lanyard was not tied off to an anchor point.
