Incident Narrative

An employee was on a lift approximately 25 feet in the air, working to pull an air conditioning unit from the top rack of the warehouse. The employee fell to the cement floor, resulting in a brain bleed, injury to his left eye, multiple fractures on the left side of his face, a fractured jaw, and six fractured ribs. The employee was not wearing fall protection at the time.
