Incident Narrative
An employee was tripping out pipe on a water drill rig. As he was breaking the drill pipe loose, a bolt on the snub line slipped out of the tong clamp which holds the pipe. The tong spun and struck the employee in the back of his upper right shoulder and pushed him into the driller's console with knobs to operate levers. The employee sustained fractures and a mouth injury.
