Incident Narrative

An employee was descending a roof access ladder after replacing air handler filters on the roof. He lost footing on the ladder rung and fell backward. He sustained multiple facial lacerations, a facial fracture, a laceration to the back of his head, a nasal fracture, left lower extremity lacerations, and a lacerated spleen.
