Incident Narrative

An employee was fixing and replacing portions of the chimney on a residential (townhouse) building. The employee was setting up scaffolding when they fell off the scaffold, struck the roof/gutter, and then fell to the ground. The employee was hospitalized with three fractures in his neck, three fractures in his back, and a fractured left wrist.
