Incident Narrative

An employee was climbing a ladder to begin stripping a mold of precast concrete from its form. The employee was approximately 6 feet above ground when he stepped off the ladder sideways onto the top of the mold. The ladder gave way beneath him, and he fell backward from the form onto the ground striking his head on the ground. The employee was hospitalized for a concussion.
