Incident Narrative

An employee was using a gantry crane to move a copper cylinder across a room. As the employee went to stop the cylinder from swaying, the cylinder shifted on its holder and the employee's left little fingertip was crushed between the metal cylinder and the support at the bottom, resulting in a fingertip amputation.
