Incident Narrative

A maintenance employee was performing repairs on a CNC machine. The automatic tool changer (ATC) door next to the tool arm was misaligned, and the employee was removing it. The employee manually turned the tool arm which caused the door to actuate and shut suddenly pinching the employee's left hand and causing a partial amputation of the distal phalanx of the middle finger.
