Incident Narrative

On March 17, 2023, an employee was driving a truck that was hauling a bomb cart containing a 20-foot tank to be loaded onto a cargo ship. The employee positioned the bomb cart under a gantry crane which then hooked onto the tank and lifted it up about 3 feet. The tank's corner section snagged the bomb cart, which lifted the truck with the employee inside its cab. As the crane was lowering the tank, the corner of the tank dislodged from the bomb cart. The cart and the truck dropped back to the ground and the employee's head hit a metal part of the cab . The employee sustained a laceration to the back of the head and a fracture to the pelvis that required hospitalization.
