Incident Narrative
On 3/24/2021 at 4:01 p.m., an employee was on a ladder fixing an overhead condensing unit when the employee stepped onto a nearby stainless steel table and the table flipped over, causing the employee to fall 4 feet. The employee suffered injuries to their right rib area. On 5/7/21 at 8:59 p.m., an employee was checking sprocket teeth for the chain underneath a bread slicer. The employee lost their balance and the tip of their left index finger got caught between the chain and sprocket, resulting in a fingertip amputation.
