Incident Narrative

An employee was emptying out a cargo van full of cleaning equipment. He climbed up a six-foot A-frame ladder to put some personal belongings up in the loft area of a garage. He fell off the ladder onto the concrete floor and struck his head, resulting in an eardrum rupture, head trauma, and rib fractures.
