Incident Narrative

An employee was delivering a pallet of solar panels that was on top of a pallet jack on a lift gate. The pallet weighed more than 1,000 pounds. When the employee jumped off the truck, the pallet tipped over and fell on top of them, resulting in a leg fracture that required hospitalization.
