Incident Narrative

On August 17, 2022, at 10:00 AM, an employee was sorting an order of 3-inch-by 20-foot angle and flat stock steel. The bundle of material was elevated about 4 feet on forklift forks. When the employee cut the banding on the bundle, the bundle rolled off the forks and landed on his left foot. The employee's left big toe was amputated and the adjacent toe was fractured. The employee was hospitalized.
