Incident Narrative

An employee had removed the blade from a vibratory plow. The blade was on a sling that was attached to the forks of a telehandler. The employee was positioning the blade to lie down on a trailer when the sling slipped off the forks and pushed the employee off the trailer. The blade then landed on the employee's right foot and fractured it.
