An employee was loading pallets onto a box truck to deliver. As a pallet was being shrink wrapped, the employee stepped backward and tripped because his foot was caught between the truck and the lift gate. He fell backward approximately 4 feet to the floor, resulting in a left arm fracture and bruised ribs.
