Incident Narrative

An employee was on the back of a lowboy trailer, guiding a 500-pound stone slab as it was offloaded from the trailer using a forklift with a boom and a stone lifter/slab clamp. The stone slipped out of the lifter and pushed the employee off the trailer, then fell on the employee's right leg just above the ankle. The employee suffered a broken leg and was hospitalized.
