Incident Narrative
Employees had just loaded a transformer onto a heavy haul trailer using a jack and slide system. They were removing a section of rail from the system, with one end of the rail chained to the forks of a forklift. While they were backing out from underneath the transformer, a 15-foot section of rail fell from the forks and hit an employee's leg. He suffered injuries to the calf and fibula.
