Incident Narrative

An employee was pushing logs into the pasteurizer bottom deck with their left hand, while their right hand was on the track on a plastic sliding door. As the employee was finishing up the task, the plastic sliding door moved, scraping the right little finger that was in the track and resulting in a fingertip amputation.
