Incident Narrative

An employee was using a pedestal grinder to grind a piece of square metal stock that was 1/4 inch in diameter by 13 inches in length. The employee placed the piece against the grinding wheel when the piece and his gloved hand were pulled into the opening between the work rest and the grinding wheel, resulting in a partial amputation of the employee's left thumb.
