Incident Narrative

Three employees were working on a high-pressure airline when the hose failed and released air pressurized at 4,000 psi. The ruptured hose struck one of the employees, who suffered a rib fracture, multiple contusions, fractures to the right middle finger, multiple lacerations, and acute hearing loss.
