An employee was working on a 10" pipe that was pressurized for a pressure test. The employee removed a flat pipe cap and pressure caused the cap to strike the employee resulting in lacerations, fractures, broken teeth, debris in their face and finger, injuries to the neck and clavicle, and a lacerated finger.
