Incident Narrative
An employee was standing on steel floor plates. The floor plates shifted under the weight of a forklift as it backed over them. The employee lost balance; his left foot slid down and became trapped between floor plates with the weight of the forklift pushing on the plates. The employee sustained the amputation of two toes.
