Incident Narrative

An employee was lowering a pallet from the second shelf of the warehouse storage rack using a forklift. The pallet struck the wire frame shelf that it was on and knocked the horizontal bar on the rack down. The employee was putting the bar back on the rack when it fell and amputated their finger against the wire frame shelf.
