Incident Narrative

An employee was removing a three-inch diaphragm pump from a storage room in the accumulator house. After placing the diaphragm pump onto the ground outside the storage room, he tripped on a board protruding out from under the right side of the accumulator house and fell to the ground knocking the pump onto his right calf muscle and crushing it.
