Incident Narrative

An employee was using a 6-foot fiberglass ladder to hang a banner above an entry door. After he completed work on the banner, he descended the ladder and was approximately four feet high when he lost his balance and fell onto the entryway. The employee sustained a fractured L1 vertebra.
