Incident Narrative

On February 7, 2022, an employee fell 52.2 inches off the top of a vehicle engine to the concrete floor while repairing a rough terrain forklift. The engine cover had been removed and the employee was standing on the top of the engine to reach and remove a pin from a lift boom cylinder. The employee suffered a fractured left hip that required hospitalization.
