Incident Narrative

An employee was unloading a trailer using a forklift. He left the forklift and removed a strap securing freight to the wall on a deck over other freight. One or more of the four beams running from one side of the trailer to the other failed, and the freight fell forward onto him. He suffered a broken fibula and tibia.
