Incident Narrative

An employee was standing next to a bundle of purlins, guiding the telehandler operator to establish fork placement. As the forks were entering the top section of purlins, the top bundle slid down off the lower bundle, landing on the employee's right foot and trapping the employee. The purlins continued to fall, landing on the upper part of the right leg. The employee sustained a crushing injury, puncture to the right lower extremity, and a muscular injury.
