Incident Narrative

On July 24, 2020, a maintenance technician was demolishing a 10-foot-high structure inside a manufacturing facility. He had removed the structure's plywood roof and was about to climb down a ladder when the fascia board collapsed and knocked him down. He fell about 10 feet, striking a table and the floor. He suffered a broken jaw and bruising to his face, shoulders, and thighs.
