Incident Narrative

An employee arrived at the shop to pick up parts. He used a lift jack to move crates, uncrated a crate, took the parts out and set them on the side. There was a heavy metal pole that is used for the lift jacks that fell off the shelf and hit his hand. He sustained the amputation of the left middle fingertip.
