Incident Narrative

An employee was removing an attachment from a skid steer. He released the locking devices of the bucket and disconnected hydraulic hoses from the machine, but the bucket did not separate completely from the machine. The employee operated the machine and raised the bucket to check if debris was preventing it from disconnecting. When he came over to check the bucket, it dropped on and broke his left foot.
