Incident Narrative

An electric pallet jack operator was traveling down an aisle attempting to pass another stopped pallet jack, when his pallet/load got caught on another pallet in a pick location. The operator lost his balance when the equipment stopped suddenly, and his hand engaged the throttle, causing the pallet jack to move forward. His right foot had slipped off the platform and became pinned between the platform and racking. The operator was hospitalized with a fractured right foot.
