Incident Narrative

An employee was starting up a reel on a wire packaging machine when a loop in the wire wrapped around his pinky finger. As the reel was turning to wrap around the drum, the loop tightened around his pinky finger and lacerated it, resulting in a pinky fingertip amputation.
