Incident Narrative
A delivery truck with a flatbed semi-trailer was parked in the yard. The back end of the trailer has a hydraulic dovetail to facilitate drive-on/drive off loading and unloading. It was in the down position. The injured employee drove a scissor lift and parked it over the tail end lip of the lowered dovetail. With the unit placed against the dovetail, the injured employee initiated testing of the pothole protection system and began lifting the basket. The delivery truck driver, unaware of the scissor lift's presence, used a remote to lift the dovetail to its travel position so he could depart. The dovetail lifted the scissor lift and caused it to tip over with the injured employee in it. The employee fell 10-12 feet to the ground and sustained a fractured pelvis.
