Incident Narrative
An employee was lowering a scaffold board using a rope and pulley assembly when the board slipped out of the rope and fell. The board caught a tangle in the rope and caused the hook end of the rope to shoot upward, where it hit the employee in the face and head. The employee suffered a probable fracture or concussion and tissue damage in the right eye area.
