Incident Narrative

An employee was in a receiving aisle doing merchandising work. At the same time, a forklift two aisles over was moving something when the forklift struck an item. The item fell onto a safe that was double stacked on a pallet. The safe then fell onto the employee. The employee was hospitalized with two fractured vertebrae.
