Incident Narrative

An employee was climbing down a 28-foot fiberglass extension ladder when the base of the ladder became unstable and slid down a small incline and out onto the street. The ladder was struck by a moving vehicle, resulting in the employee falling 20 feet. The employee suffered a fractured right hip, cracked rib, and broken nose.
