Incident Narrative
An employee was walking toward the swinging doors to access the sales floor. A nearby electric power jack was approaching at the same time. The forks of the pallet jack came into contact with the swinging door, causing it to strike the injured employee. She fell to the ground and sustained a hip fracture.
