Incident Narrative

An employee was standing next to a crate of windshields that were packed vertically. The windshields fell forward and the crate broke. The falling materials struck the employee, causing multiple pelvic fractures on both sides, a tailbone fracture, bulging discs, and lacerations to the arm.
