Incident Narrative
An employee was taking inventory of "elements" in the warehouse. The items were stacked on a pallet that rested on a shelf 38 inches from the floor. To obtain an inventory number for one of the elements, he pushed another element out of the way. The pushed element fell towards the employee, striking him in the right femur. He sustained a right femur fracture.
