Incident Narrative

An employee was setting up jack stands for a section of pipe to rest on when a contractor on site turned on a concrete pump hose that moved the jack stand out of position. The employee attempted to set up the jack stand again while a section of pipe was being positioned on it. His right middle finger was caught between the jack stand and the section of pipe, resulting in a finger amputation from the first knuckle behind the nail.
