Incident Narrative

After placing empty cardboard into the baler, an employee grabbed the baler gate handle with his right hand and began to pull the gate down to close it. While he was adjusting cardboard with his left hand, the gate closed on his hand. The left index finger was partially amputated without bone loss, and the left middle and ring fingers were fractured.
