Incident Narrative
An employee was preparing to load a delivery truck at a client's site. The employee was attempting to position a dock plate at a loading dock when the flap at the end of the dock plate became stuck. When the employee tried to free the flap, it sprung and came down on his finger, resulting in a right index finger amputation.
