Incident Narrative
On April 26, 2016, at approximately 4:00 p.m., an employee was adjusting the brush table, which is part of the equipment that loads buns into bags. To adjust the brush table, the employee loosens a lock bolt, slides the brush table forward or backward, and retightens the lock bolt. While adjusting the brush table with his right hand, the employee's right index finger was pinched between the pusher on the bagger and the brush table, amputating the fingertip.
