Incident Narrative

The injured employee was acting as a spotter in the parking lot for a lift operator that was loading a pallet of mulch into a customer's vehicle. The lift operator was positioning the load when the pallet of mulch shifted, toppled over, and struck the injured employee on the shoulder, knocking him to the ground. The employee struck his head on the pavement and was hospitalized for head trauma.
