Incident Narrative

An employee was helping to load empty 500-gallon water troughs from a forklift onto a customer's flatbed trailer. The water troughs slid from the forklift, fell onto the employee, and knocked him to the ground, resulting in shoulder and back injuries and a spinal cord contusion.
